Unit 4: The Golden Thread in Sections D and H

Any social care-related needs identified in Section D need to be linked to one or more of the outcomes in Section E, which will also link to relevant social care provision in Section H, to ensure the continuation of the Golden Thread. 

In the previous unit you had a go at determining where some recommended social care provision should be included in the example of Vikram. The below Section D and H sets out the correct answer to this, as well as demonstrating how the Golden Thread links can be presented in an EHC plan. Note that Vikram’s identified care need (and the impact of this need) is summarised in Section D and that this need has a corresponding provision in Section H. Also note that Section H clearly indicates that this provision will be contributing to Vikram achieving a related outcome.


Whilst each Local Authority EHCP template will vary in layout and format, it is still important to ensure social care needs are matched to one or more of the outcomes and that there is corresponding social care provision.
